Lebo Schools is proud to announce that Dr. Alex Boss from Osage Sports & Family Chiropractic will once again this year be offering his service for sports physicals. Dr. Boss will be at the school on May 10th from 8:30-11:30 to perform sports physicals for the next school year. This is offered to all 5th thru 11th grade students. The cost is $10 and can be paid by cash or check made out to Osage Sports and Family Chiropractic. Physical forms will be sent home or can be picked up in the high school office. All physical forms need to be completed and signed by the athlete and parents (check every page because there are several places to sign).
If you choose to have your family physician perform your child's physicals, please make sure you use the official KSHSAA form. Forms can be found on the KSHSAA website or we have copies in the HS office.
All physicals must be completed before the student-athlete can start practice in August.
